The Glendale Police Department has issued a silver alert for missing 64-year-old Anthony Gardner.

Gardner was last seen Oct. 2 leaving Aurora Sinai Hospital after being transported.


Officials say he left the hospital around 2:30 a.m. and went to his group home at Wisconsin Community Services to pick up a check and never returned.

Police say he was last seen wearing a Green Bay Packers fleece, red and black checkered pajamas, and white tennis shoes.

Gardner is described as being 6 feet, 1 inch tall, weighing around 260 pounds. He has brown eyes and a black beard.
